How IPMI works: bills, claims, renewal and moving countries

From treatment approval to payment
Direct billing means the insurer pays the provider. Reimbursement means you pay first and submit the claim. Neither method removes the deductible, an exclusion or a benefit limit. Ask for approval of the treatment and a clear statement of your share before non-emergency care.
BDIL Pro, March 2026, article 11 requires advance authorization for listed services and coordination for gene therapy/CAR-T. Article 12 describes claims and supporting documentation. Keep itemized invoices, medical reports, receipts and the insurer’s written response.

A policy maximum is not a treatment budget
Global Care Pro has a US$5 million annual maximum, but gene therapy and CAR-T share US$1 million per person per policy year. You cannot add the same shared benefit twice or substitute the policy maximum for it. Authorization and article 10.5 experimental-treatment exclusions remain relevant.
The reviewed BDIL benefit tables state a 40% penalty without authorization, while article 11 of the conditions says listed services are not covered without it. The consequence is unresolved: we do not assume that paying a penalty buys coverage. Get written clarification before treatment.

What can remain on your bill?
A deductible, coinsurance, out-of-network payment reduction and a treatment sublimit can operate together. Do not assume that your coinsurance cap also limits excluded charges or amounts above a benefit limit. Where a percentage is uncapped, there is no fixed maximum for that percentage alone.
BDIL Pro defines an individual deductible per policy year. Its definition allows a different amount in the US: if you already met a lower home-country deductible, you can still owe the difference. Specific carryover and waiver rules in article 5 do not turn the deductible into a reimbursable expense.

Can you keep renewing if you become ill?
Guaranteed renewability protects the ability to continue cover, not a fixed premium. The BDIL Pro March 2026 contract provides automatic renewal when the full premium is paid, while retaining fraud, false-statement, omission and eligibility exceptions. There is no maximum renewal age for the primary insured or spouse/partner; children have separate dependent-eligibility rules.
Article 13.19 permits rate changes for the group at renewal, but not an individual increase based on your claims history. This does not make every illness covered: the accepted terms, benefits and exclusions still matter.

What can change at renewal?
The reviewed BDIL definition of Renewal Date allows review or adjustment of premiums, benefits and coverage options. Article 13.19 rules out an individual claims-based premium penalty but allows general rate changes. Compare the renewal schedule and amendments, not only last year’s bill.
Payment matters too. Article 13.17 grants a 30-day grace period; if the full premium is not paid, cover ends at the end of the period paid for. Claims during the grace period are payable only if the full premium is paid before it expires. Reinstatement after cancellation is not automatic.

Do you have to live abroad? What if you move?
You do not have to live outside Mexico for international insurance to be useful. But living in Mexico does not, by itself, establish eligibility to buy any foreign policy. Residence, the issuing company and the proposed transaction all matter.
Being able to receive treatment in a country and being able to live there on the same policy are different things. BDIL Pro requires non-US residence at application. Becoming a US resident ends cover at the next renewal under its eligibility provisions.
Article 13.6 requires written notice of a residence change within 30 days. The insurer may change the premium, deductible or plan, or cancel the policy. Failing to report the move can also lead to a change or cancellation. Continuity is not automatic.

An extended stay can become a residence question
BDIL defines Country of Residence using where you spend most of a calendar or policy year, or more than 180 continuous days in any 365-day period. Its separate US-resident definition also considers making the US your long-term or principal home. Do not treat 180 days as a universal safe allowance for US stays.
The consequence depends on the specific move, not simply where you buy treatment. Report it before relying on continued cover. The reviewed contract does not establish a universal list of acceptable replacement countries or guarantee the same price after moving.

Worldwide care still has a map and payment rules
Outside the US, Global Care Pro permits free provider choice, which can support care in Europe or Latin America. Within the US it uses GlobalCare Network. Medical Select International Pro excludes the US and pays 60% in its Premium Providers network elsewhere. Geographic reach does not erase provider rules, sanctions exclusions, deductible or authorization requirements.

If a treatment is denied, where can you challenge it?
Identify the company that issued the contract and ask for the denial in writing, including the clause relied on. Keep the policy, endorsements, approvals and receipts. The brand and treatment location do not determine the applicable law or complaint route.
BDIL Pro, March 2026, specifies binding arbitration in article 13.21 and Bermuda law. That differs from a Mexican-issued policy. For institutions within CONDUSEF’s remit, the insurer’s specialized complaints unit (UNE) and CONDUSEF complaint mechanisms may be available; check the entity in its records. We do not assume those routes apply to every foreign issuer.

What does arbitration mean in practice?
BDIL Pro article 13.21 sends covered disputes to binding arbitration under the specified Bermuda legislation and applies Bermuda law. This is a process before arbitrators, not a promise that a local court where treatment took place will decide the claim. The arbitration is seated in Bermuda and conducted in English. The clause initially allocates each party’s own representation and appointed arbitrator, plus half the third arbitrator and arbitration expenses; the tribunal can determine the final allocation.
CONDUSEF: UNE y atención a usuarios
What if you already have insurance?
Compare what you could gain, what you would pay and what you might lose. A new insurer may exclude an already accepted condition, review your medical history again or apply new waiting periods. Recognition of prior cover does not preserve every existing right.
Changing the broker on the same policy is different from replacing the insurance contract. Before cancelling, obtain written acceptance, exclusions and start dates for the new cover. Two policies do not automatically coordinate every payment, and one does not necessarily pay the other’s deductible.

How two policies interact
BDIL Pro articles 13.7–13.8 require disclosure and supporting payment records for other cover. The insurer determines which amounts can count toward its deductible and describes when it pays second. Total payments cannot exceed covered expenses. This is a specific coordination process, not a promise that two policies pay every gap.

Common questions
Is direct billing the same as full payment?
No. It identifies who pays the provider first. Deductibles, limits and excluded expenses can still leave you with a bill.
Can a US-covered policy remain unchanged after a US move?
Not automatically. The reviewed BDIL contract ends cover at the next renewal when the insured becomes a US resident.
